Postmortems
Post-incident analyses for organizational learning and recurrence prevention.
Why do we do postmortems?
Postmortems are blameless — the goal is to understand systemic causes, not to point fingers. Every P1/P2 incident must generate a postmortem within 5 business days of resolution.
How to create a postmortem
- Copy the postmortem template
- Name the file
YYYY-MM-DD-short-title.md - Fill it in within 5 business days of the incident
- Share with the team via Pull Request for collective review
